DescriptionThe Community Health Worker (CHW) works collaboratively
with care team to implement a person-centered plan of care, which
helps families navigate access to community services and resources
and assessing ways to overcome barriers that affect patient's
health. As a Community Health Worker you will:
- Implement a person-centered plan of care through conducting
regularly scheduled home visits to assist homebound patients and
families
- Assist patients and families with access to community services
and resources
- Provide intensive support, advocacy, and empowerment while
addressing social barriers that affect patient's health
- Encourage high-risk patients and families to engage with their
VPA provider to effectively manage their chronic disease as well as
participate in preventive careQualifications
- High school diploma or equivalent
- At least 1 year of experience working with community agencies
providing services encompassing the basic competency areas of
health care (community resources, communication skills, individual
and family advocacy)
- Familiarity with the resources available within the
community
- Valid driver's license and automobile insurance is
