  • Largest home-based primary care practice in the US for over 28 years, making a huge impact in healthcare today!Position DescriptionThe Community Health Worker (CHW) works collaboratively with care team to implement a person-centered plan of care, which helps families navigate access to community services and resources and assessing ways to overcome barriers that affect patient's health. As a Community Health Worker you will:
    • Implement a person-centered plan of care through conducting regularly scheduled home visits to assist homebound patients and families
    • Assist patients and families with access to community services and resources
    • Provide intensive support, advocacy, and empowerment while addressing social barriers that affect patient's health
    • Encourage high-risk patients and families to engage with their VPA provider to effectively manage their chronic disease as well as participate in preventive careQualifications
      • High school diploma or equivalent
      • At least 1 year of experience working with community agencies providing services encompassing the basic competency areas of health care (community resources, communication skills, individual and family advocacy)
      • Familiarity with the resources available within the community
      • Valid driver's license and automobile insurance is requiredWon't you join us?
