you will contribute: The Clinical Supervisor works in close
collaboration with the Manager, Clinical Operations and is
responsible for ensuring the delivery of high?quality,
patient?centered hospice care across one or more Interdisciplinary
Teams (IDT). This role provides leadership, clinical oversight, and
operational support to nursing and clinical staff, ensuring that
care is delivered in accordance with regulatory standards,
organizational policies, and the individualized needs of each
patient. The Clinical Supervisor oversees the planning,
implementation, and evaluation of clinical services through
comprehensive caseload assessment, monitoring of clinical quality,
and optimal utilization of personnel. This position plays a key
role in maintaining clinical excellence, fostering strong team
performance, and supporting a positive patient and family
experience. This role requires compassion, strong clinical
judgment, excellent communication skills, and the ability to
support patients and families through highly sensitive and
meaningful experiences. How you will make an impact: Provide social
casework as an entity, where appropriate, to individuals and
families receiving nursing or other services from the Agency
Performs documentation reviews of clinical staff regarding direct
patient care in the IDT(s) they oversee and report results to the
Manager- Clinical Operations Completion of in-home annual
competency and evaluations for IDT clinical staff Facilitates case
management process with staff members, physicians and referral
sources Maintains open lines of communication with all members of
the continuum of care team Conduct ride-along with all clinical
staff to ensure patient needs and hospice practices are being met
as business needs arise Educating staff on processes, procedures
and policies as needed Assign and monitor patient care schedules to
ensure continuity and quality regulations Monitor to ensure
coordination and follow-through is complete for all referrals to
the hospice program Assist in the coordination and assignment of
all newly admitted hospice patients to the appropriate team and/or
disciplines Ensure communication of all patient admissions and
changes in Plan of Care is appropriately relayed to each member of
the Interdisciplinary Team and Primary Care Physician Participating
in routine visits and admissions of patients in times of high
census or low staff availability Selects and maintains a qualified,
well-organized staff to provide care for the needs of the patients
Is accountable and supervises the IDT team i.e. RN's, LPN's,
Medical Social Workers, Chaplin’s, and Hospice Aides Available to
work after-hours to provide support and/or conduct in-person visits
Coordinate with the hospice physician to facilitate and run the IDG
meetings in the absence of and/or in support of the Manager-
Clinical Operations. The Experience and Skills You’ll Bring Active,
unrestricted RN license Excellent verbal and written communication
skills The ability to assess patient needs and formulate
individualized patient care plans to meet those needs Proficiency
in personal computer use, including e-mail, clinical, word
processing, and spreadsheet software Must maintain a valid driver’s
license and good driving record 3 or more years of nursing
experience Preferred Knowledge, Skills and Experience: Two years’
supervisory experience in an in-patient hospital, home care agency,
or residential facility Experience within Hospice
