Job Description:

agencies. We are seeking a dedicated Certified Case Manager to
enhance the efficacy of our integrated patient care services. This
role involves coordinating comprehensive plans that address the
complex medical and psychosocial needs of our patients, ensuring
their smooth transition between care settings, and facilitating
overall patient wellness and autonomy. This professional will play
a pivotal role in representing Encompass Health's mission to
provide compassionate, innovative, and exceptional care. Assess new
and ongoing cases to determine appropriate level of care and
services Develop, implement, and evaluate individualized patient
care plans Coordinate with healthcare professionals to streamline
care transitions and share pertinent patient information Advocate
for patients' needs and liaise with families to include them in the
care process Monitor patient progress and coordinate with
healthcare providers to modify care plans as necessary Ensure
compliance with all federal, state, and local regulations as well
as with Encompass Health's standards and policies Strong
interpersonal and communication skills Proficiency in healthcare
management systems and software In-depth knowledge of healthcare
regulations and case management protocols Critical thinking and
problem-solving skills Ability to multitask and prioritize in a
dynamic environment Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Certified Case Manager (CCM)
credential mandatory Licensure as a Registered Nurse (RN) or
Licensed Social Worker (LSW) is advantageous
