Job Description Job Description JOB TITLE: HOME HEALTH CARE, REGISTERED NURSE (WAYNE/OAKLAND COUNTY) HIRING IMMEDIATELY We are a growing company looking to expand our field staff in Wayne, Oakland, and Macomb County. Ability to make your own schedule and competitive pay. Essential Functions: *Performs assessments and procedures, which require substantial specialized knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences. *Visits in home clients to evaluate the quality of care being provided and reviews clinical notes. *Completes nursing initial and ongoing assessments of all in home clients within specified deadlines. *Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s). *Counsels and educates the patient/client and family in meeting healthcare related needs. *Supervises paraprofessional and licensed professional team members. Nurse to admit patients into skilled home health care services. May perform Start of Care only or continue care with weekly follow-up appointments. Ability to use Kinnser EMR Software. Understanding of Medicare regulations and guidelines pertaining to home health care services in the patient's home. Will supervise LPN's and Home Health Aides Compensation: $65-100 per visit
